The challenge

Four websites, and no two of them selling to the same Google.

An orthopaedic and spine clinic needed to be found by people searching for a knee or a spine specialist in their own city — a small number of searches a day, every one of them close to booking. An energy modelling and green building consultancy sold LEED certification work to architects and developers in the United States and needed to rank in a market it had no physical presence in. A multi-brand appliance service network had national coverage and almost no organic footprint. A film and entertainment publisher had a large content library and needed the library to actually get indexed, ranked and read.

The failure mode an agency reaches for here is a single blended figure — total clicks, average growth, one percentage across four accounts. It would have been a very good-looking number in this case, and it would have hidden every decision worth making. The publisher’s volume alone would have swallowed the other three whole.

What we did

Reported every account on its own line. No blended traffic figure, no agency-wide growth percentage. Each account is judged against the search market it actually competes in.

Fixed the foundation before the content. Crawlability, indexation, speed and schema first on every one of the four — a page Google struggles to read cannot be optimised into ranking.

Built the keyword set around intent, not volume. For the clinic that meant condition-and-city searches with a booking behind them, not the high-volume medical terms it would never win and could not convert.

Treated the international accounts as international. The consultancy’s target market is American; its keyword research, content and link work were built for US search from the start rather than for a home-market footprint pointed at an export audience.

Grew authority deliberately. Referring domains went from a standing start to 254 on the consultancy, 256 on the service network and 1,200 on the publisher — earned through relevant placement, not bought in bulk.

Published against the library the publisher already had. Existing content restructured, internally linked and made indexable before a single new article was commissioned.

Reported rankings, traffic and position together. An account can grow traffic 184% and still sit at average position 30 — and knowing both at once is the difference between a plan and a press release.

The result

The publisher took 425,000 clicks from 56.8 million impressions over twelve months at an average position of 8, and its Semrush footprint reached 12,400 ranking keywords and 28,600 organic visits a month, up 173% and 178%. Its traffic chart has no step in it anywhere — twelve months of unbroken climb, which is exactly what content SEO is bought to produce.

The energy consultancy posted the best position in the study, 4.2, at a 2.3% CTR on 218,000 impressions, with 11.2K monthly organic traffic (+186%), 2,800 keywords (+167%) and an authority score of 80. 91% of that visibility is in the United States — a consultancy whose search visibility sits almost entirely in a market it sells into from outside.

The clinic reached 2,840 clicks at an average position of 6.5 with 589 monthly organic visits, up 105%, on 107 keywords and an authority score of 9. Small numbers, and the right ones: this is a local practice nine months into an engagement, and a knee-pain search that ranks sixth in its own city is worth more to it than a thousand impressions from three states away.

The appliance service network is the account that makes this study honest. Organic traffic up 184% to 4.2K a month and 1,800 keywords ranking — while Search Console still reports 584 clicks at average position 30.4 and a 1.1% CTR. Both readings are correct. The keywords are landing; most of them are landing on page three, where almost nobody clicks. The work in front of that account is not more keywords, it is moving the ones it already has from position 30 to position 10, and saying so is more useful than quoting the 184%.

No total. The publisher's twelve-month window and the clinic's six are not the same measurement, and one of these accounts is a content library and another is a single-location clinic. Adding the click columns would produce a number that describes nothing.

Read the CTR column against the position column rather than on its own. The publisher's 0.7% is the lowest rate here and by far the largest click count — 56.8 million impressions at position 8 is a different business from 165,000 at position 6.5.

Authority score is Semrush's own 0–100 estimate, not a Google metric. It is in the table because it explains the gap between the clinic and the consultancy: 9 against 80 is why one of them competes on 107 keywords and the other on 2,800.

Where the traffic comes from

Two of these accounts sell locally and two sell across borders, which changes what the SEO is even for.

The clinic is absent from this table on purpose: its Semrush country row reports visibility rather than a traffic share, and the two are not interchangeable. A local clinic's real distribution question is which neighbourhoods appear in the map pack, which is a Business Profile report, not this one.
